Learning objectives

After the webinar, participants will learn on:

  • innovative aims and outputs of the I-CARE4OLD H2020 project and the existence of the prognostication platform developed for clinicians to estimate the therapeutic outcomes of proposed interventions
  • new methods of testing intervention outcomes in complex older patients with multimorbidity, polypharmacy, disability and frailty will be introduced, as well as methods of testing of deprescribing strategies
  • results of two concrete machine learning studies testing real outcomes of a new use or deprescribing of anticholinergics or antipsychotics in older nursing home residents and helping to estimate individual treatment effect of these interventions will be presented

 

Description

The aim of this webinar is to inform participants on main outputs of the ongoing I-CARE4OLD H2020 project (2021-2025) that for the first time developed ML (machine learning) techniques enabling to test real outcomes of various pharmacological and non-pharmacological interventions in complex older adults by using pharmaco-epidemiological data of millions of seniors assessed by interRAI comprehensive geriatric assessment (CGA) tools. New ML methods help to overcome limits of randomized controlled trials that were not able to evaluate real medication outcomes in older adults due to high complexity- frequent multimorbidity, polypharmacy, disability and frailty. AI studies dealing with the real efficacy and safety of antipsychotics and anticholinergics in older patients in nursing homes will be presented, as well as real outcomes of deprescribing of these medications. Newly developed WP7 I-CARE4OLD H2020 prognostication platform will be introduced, helping clinicians to estimate the real individual patient outcomes of indicated interventions.

 

Hein van Hout is an expert in geriatric and primary care research at the Amsterdam University Medical Center. His research primarily focuses on improving the quality of life and care for vulnerable older adults and their informal caregivers. This involves developing decision support tools for healthcare professionals, identifying best practices in care organization, and rational (de)prescribing. Professor van Hout published over 200 international peer-reviewed papers and coordinated several large multinational studies, including the IBenC project on home care quality and cost, the PACE trial to enhance palliative care in residential homes, and the I-CARE4OLD project aimed at improving prognostics for older individuals with chronic complex conditions. He is board member of interRAI, a global network dedicated to improving the quality of life for vulnerable populations through evidence-based clinical practice and policy decisions. His work is instrumental in advancing the field of geriatrics and primary care.

Rosa Liperoti is associate professor of geriatrics at Università Cattolica del Sacro Cuore, Rome (IT) and director of the Geriatric Rehabilitation Division at Fondazione Policlinico Universitario A. Gemelli IRCCS, in Rome (IT). Prof. Liperoti has extensive research background focusing on dementia, frailty, multimorbidity, and geriatric pharmacoepidemiology, being part of numerous national and international studies dedicated to the investigation of health outcomes associated with pharmacotherapy among older adults with chronic complex conditions. She is author of over 120 scientific publications including research articles in peer-reviewed journals and book chapters.

Daniela Fialová is associate professor at the Dept of Social and Clinical Pharmacy, Faculty of Pharmacy (FoP) and Dept of Geriatrics and Gerontology, 1st Faculty of Medicine, Charles University (CUNI). By clinical background she is clinical pharmacist trained in acute geriatric wards and having experiences also in ambulatory and home care with monitoring and resolution of drug risks in older adults. She chairs Research Unit “Ageing, Polypharmacy and Changes in the Therapeutic Value of Drugs in the AgeD” and the University Educational Centre in Clinical Pharmacy (FoP CUNI). She collaborated on over 10 international projects, e.g. the ADHOC (2001-2005), SHELTER (2009-2014), PREDICT (2009-2011), EU COST Action IS1402 on Ageism (2015-2018), EuroAgeism H2020 (2017-2022), I-CARE4OLD H2020 (2021-2025), INOMED (2017-2021) and NETPHARM (2021-2028).
