Learning objectives

After the webinar, participants should be able to:

  • understand how pharmacists can improve the safety of drug therapy of patients with impaired renal function.
  • understand how the glomerular filtration rate (GFR) can be determined and which parameters must be taken into account, which estimation formula is suitable for dose adjustment to renal function and which drugs are particularly affected by dose adjustment to renal function.
  • identify essential steps and stakeholders involved in establishing a renal pharmacist position.

This seminar will use patient cases as examples to show how these drug-related problems can be recognized and solved in patients with renal insufficiency.

 

Description

The aim of the webinar is to demonstrate how to establish the role of a renal pharmacist within a hospital setting. Through practical insights and real-life case example, participants will gain a deeper understanding of the pro’s and con’s of calculating GFR with different methods and on how to adjust common drugs appropriately in acute and chronic kidney injury. It also addresses implementation strategies, interdisciplinary collaboration, and the clinical impact of specialized pharmacy services in nephrology.

 

Andreas von Ameln-Mayerhofer studied pharmacy at the University of Freiburg, Germany 1990-1995 and completed his doctoral thesis at the University of Tuebingen, Dept Neuropharmacology. In 2011 he started to work in Sindelfingen as Clinical Pharmacist and obtained his specialisation. He quickly became an Antibiotic Stewardship Expert and is head of the interdisciplinary AMS-Team in the hospital group. As one of the main achievements, he established Therapeutic Drug Monitoring of anttinfectives for critical patients. Since 2016, he is ID-Pharmacist. Additionally, he attends regular visits on the geriatric traumatology ward in Sindelfingen and started the Renal Pharmacist project on this ward. Since 2022, Andreas is head of department and member of the Intensive Care Medicine and Antiinfectives council of the German Association of Hospital Pharmacists (ADKA).
Currently, he is serving as Lecturer and Guest Scientist at the University Clinic Tuebingen, Department of Clinical Pharmacology.

Ina Richling is a specialist pharmacist in clinical pharmacy, infectious diseases, and medication management in a hospital setting. She studied pharmacy at the University of Bonn from 1995 to 2000 and earned her Doctor of Pharmacy (PharmD) from the University of Florida, Gainesville, between 2007 and 2010.
She is a board member of the Westphalia-Lippe Chamber of Pharmacists and the Pharmaceutical Network Europe and holds a teaching position at the University of Bonn. Additionally, she is actively involved in the German Society of Internal Medicine (DGIM), contributing to the Commission for Drug Therapy Management and Medication Safety.
On behalf of the Drug Commission of the German Medical Association, she has contributed to the development of the S3 guideline on polypharmacy and to the revision of the National Care Guideline on Chronic Coronary Heart Disease (CHD) within the “Drug Therapy” working group.
She also serves on the board of the Initiative for Pharmaceutical Care, is a scientific advisory board member of the German Society of Clinical Pharmacy and is part of the German Association of Hospital Pharmacists (ADKA) committee on pharmaceutical interventions. She leads the ADKA working group on medication-related discharge management.
As co-initiator and tutor of the ATHINA project for medication safety in community pharmacies, she is committed to improving pharmaceutical care. She is also editor of the book “Medication Review – Fundamentals and Case Studies.”
