  • Professor in Medication Safety and Chief Pharmacist, Department of Clinical Pharmacy and Toxicology, Maastricht University Medical Center, Maastricht, the Netherlands.

Learning objectives

By the end of the webinar, participants should be able to:

  1. Describe the different types of prescribing cascades and why they matter.
  2. Consider practical steps that can be incorporated into practice to help minimise prescribing cascades for patients.
  3. Critically evaluate and discuss patient cases with prescribing cascades.
  4. Enhance skills in identifying and preventing prescribing cascades and generating recommendations to manage inappropriate prescribing cascades.

Description

To share evidence and experiences regarding strategies to help identify, manage, and prevent prescribing cascades.

Dr Kieran Dalton has been a lecturer in clinical pharmacy in University College Cork, Ireland since 2019, where he is actively involved in research and teaching undergraduate and postgraduate students across healthcare disciplines. He has a wealth of personal experience in working as a pharmacist in hospital, community, and long-term care settings, as well as in supervising pharmacists undertaking PhD and Masters degree research in a variety of clinical practice settings.

From 2016 to 2019, he completed his PhD on investigating factors affecting the implementation of recommendations to optimise medication appropriateness in hospitalised older adults. Since then, Kieran has continued to conduct research on medication optimisation in older adults, with a particular interest in prescribing cascades, deprescribing, and pharmacist interventions.

Kieran was part of an international multidisciplinary project that focused on exploring how sex and gender may influence the incidence of prescribing cascades (iKASCADE). This collaboration has been involved in developing tools to aid prescribing cascade recognition, including ThinkCascades and the Potentially Inappropriate Prescribing Cascade (PIPC) list. Kieran’s research has also included exploring the views of healthcare professionals to identify barriers and facilitators to prescribing cascade identification and management, ultimately aiming to help identify better strategies to minimise their prevalence in practice.

Prof.Dr. Fatma Karapinar is a hospital pharmacist-epidemiologist and Board Certified Geriatric Pharmacist (BCGP, an American certification for pharmacists in geriatric care). She is the head of the Department of Clinical Pharmacy and Toxicology at Maastricht University Medical Center and is Professor in Medication Safety at the Cardiovascular Research Institute Maastricht (CARIM) of Maastricht University in the Netherlands.
Her research focuses on transitional care around hospital discharge, identifying and preventing harm due to medication (e.g. readmissions, prescribing cascades, adverse drug events) and pharmacoepidemiological research using hospital and primary care data.

Specifically on prescribing cascades, she has published multiple papers using prescription sequence symmetry analyses to identify cascades in large datasets. Furthermore, with her team, she develops fact sheets per prescribing cascade (the CASC-ADE tool) to help healthcare professionals to identify cascades and manage them. Finally, she has developed algorithms in pharmacy information systems to systematically identify cascades in real time.

She is also involved in the development of hospital pharmacy, research, and professional collaboration. She contributes to the European Association of Hospital Pharmacists (EAHP) as a member of its Scientific Committee and is actively involved in strengthening academic and practice-based pharmacy networks in the Netherlands. In this context, she chairs ZANA (Southern Academic Network of Pharmacists), contributes to the Scientific Advisory Board of KNMP (Dutch Pharmacy organisation), and serves on the board of PRISMA (Practice Research in Collaboration with Pharmacists).
